What Is Medicare Part D?

Medicare Part D is prescription drug coverage offered through private insurance companies approved by Medicare. It helps pay for medications you take at home — not drugs given in a hospital or doctor's office, which are typically covered under Part A or B.

Part D is available in two ways:

Standalone Part D Plan

A separate drug plan that works alongside Original Medicare (Parts A & B) or a Medicare Supplement plan. You choose a plan based on your specific medications.

Bundled in Medicare Advantage

Most Medicare Advantage (Part C) plans include drug coverage. If you choose Medicare Advantage, you typically don't need a separate Part D plan.

The Part D Late Enrollment Penalty — Don't Make This Mistake

If you don't sign up for Part D when you're first eligible and don't have other creditable drug coverage, you'll pay a permanent late enrollment penalty for as long as you have Medicare drug coverage.

The penalty is calculated as 1% of the national base beneficiary premium for every month you went without coverage. This amount is added to your Part D premium every month — for life.

How Part D Coverage Works

Part D coverage has different phases. Understanding them helps you plan for your drug costs throughout the year.

Deductible Phase

You pay 100% of drug costs until you meet your plan's deductible (up to $590 in 2025). Not all plans have a deductible.

Initial Coverage Phase

You pay your plan's co-pays or co-insurance for covered drugs. Your plan pays the rest.

Catastrophic Coverage Phase

Once your out-of-pocket costs reach $2,000 (in 2025), you pay nothing for covered drugs for the rest of the year.

How to Choose the Right Part D Plan

What tier are your drugs on? (Lower tiers = lower cost)
What are the co-pays or co-insurance for your specific medications?
